7	Any paperwork pertaining to any Lawsuits/Judgment(s)/Garnishment(s)
8	Divorce Decree if divorced within past 3 years
9	$1,200.00 cash or money order, as a deposit on the legal fee and Court filing fee
10	And complete my 4 page Bankruptcy Questionnaire (PDF Format - Adobe Acrobat Reader). We will go over it completely during the consultation.
